Periodontitis is a bacterial inflammation of the periodontium, which affects the gums, jawbone, and fibers that anchor the teeth in the bone. The term is often used synonymously with periodontal disease, although strictly speaking, periodontal disease describes the advanced, destructive course of the disease. In our practice, we distinguish between reversible gingivitis, a superficial inflammation of the gums, and more profound periodontitis, which already attacks the supporting tissue of the teeth.Periodontitis usually begins as gingivitis due to bacterial plaque on the gum line. Without appropriate treatment, the bacteria penetrate deeper into the tissue and form so-called gum pockets. These bacterial niches can no longer be reached with home oral hygiene. Our dentist Dr. Niels Hoffmann will be happy to explain the exact difference between the various stages of the disease. Timely periodontal treatment can prevent the transition from reversible gingivitis to irreversible periodontitis, thus maintaining your dental health in the long term.
The main cause of periodontitis is bacterial deposits, also known as Plaque, which accumulate at the gum line. These deposits are caused by poor oral hygiene and harden over time to form calculus. Certain types of bacteria in Plaque produce toxins that damage the gum tissue and trigger inflammatory reactions.
Various risk factors contribute to the development of periodontitis. Smoking is one of the most important risk factors, as nicotine impairs blood circulation in the gums and weakens the immune system. Diabetes mellitus also significantly increases the risk of periodontitis. Stress, hormonal changes during pregnancy or menopause, and genetic predisposition can also contribute to the disease.

Periodontitis is diagnosed through comprehensive diagnostics in our practice. Our dentist, Dr. Niels Hoffmann, first measures the depth of the gum pockets with a special probe. Healthy gum pockets are a maximum of 3 mm deep. Deeper pockets indicate Periodontitis. In addition, we assess gum bleeding and tooth loosening and, if necessary, take X-rays to document bone loss.
Modern periodontal treatment is minimally invasive and usually painless. The therapy focuses on the mechanical removal of all bacterial plaque and calculus from the tooth root surfaces. This is done with special hand instruments and ultrasonic devices. For deeper pockets, local anesthesia can make the treatment more comfortable.
In severe cases, we supplement mechanical therapy with additional measures. These may include antibiotic rinses of the pockets or, in special cases, the administration of antibiotics. Preparation for treatment
Before the actual periodontal treatment, we carry out a detailed preliminary examination. This includes measuring all pocket depths, photographically documenting the initial findings, and explaining the planned course of treatment. We draw up a detailed treatment plan and discuss the expected costs of periodontal treatment with you.
An important part of the preparation is professional teeth cleaning. We remove all soft and hard deposits above the gum line and polish the tooth surfaces. At the same time, we train you in optimal oral hygiene measures, as your cooperation is crucial for the success of the treatment. Dr. Niels Hoffmann's team will explain the correct toothbrushing technique and the use of dental floss or interdental brushes.
In extreme cases, severely loose teeth or teeth that are not worth preserving may be removed before the main treatment. This creates better conditions for the healing of the surrounding tissue and facilitates oral hygiene at home.
Carrying out the treatment
The main treatment for periodontitis is systematic and thorough. Under local anesthesia, we clean the root surfaces in the deepened gum pockets with special hand instruments. In doing so, we remove all bacterial plaque, calculus, and inflamed tissue. Modern ultrasonic devices support the mechanical cleaning and simultaneously rinse the pockets with disinfectant solutions.

Additional measures may be necessary for particularly deep pockets or stubborn inflammation. These include the local application of medication or, in special cases, systemic antibiotic administration. Germ tests can be carried out using pool samples to determine the specific cause.
In many cases, the Clean&Seal procedure is useful for gently removing biofilms and inflamed tissue and then protecting against new infections.
Follow-up treatment
Follow-up treatment is crucial for the long-term success of periodontal treatment. After about 6-8 weeks, we check the healing process and measure all pocket depths again. In most cases, the signs of inflammation have decreased significantly and the pockets have become smaller.
If deep pockets still exist or new signs of inflammation appear, we will carry out follow-up treatment. This may include renewed mechanical cleaning or, in rare cases, surgical measures. Dr. Niels Hoffmann will discuss all necessary steps with you in a transparent manner.
In the long term, regular, supportive periodontitis therapy is necessary. These professional teeth cleanings should be performed every 3-6 months, depending on individual risk. This is the only way to prevent bacterial plaque from accumulating again in the treated areas. Our prophylaxis specialists will create an individual preventive care plan for you.

After periodontal treatment, you should refrain from sports and physical exertion for 24 hours. Rinse your mouth carefully with lukewarm water or a recommended mouthwash. Avoid hard foods and smoking. Careful oral hygiene is crucial for successful healing.
Professionally performed periodontitis treatment has only minor risks. Temporary tooth sensitivity or slight swelling may occur. In rare cases, secondary bleeding or infection may occur.
